What happened?
Duet: Partners In Health & Aging has moved forward with a mission-aligned program transfer intended to preserve continuity of Duet’s three core service lines: homebound adult services, family caregiver services, and kinship care services.
Why did Duet make this transition?
This was a proactive, mission-driven decision to ensure the long-term sustainability of these vital aging services. It reflects responsible leadership and forward-thinking stewardship to position the three core service lines for long-term stability and growth.
What happens to Duet?
Duet’s legacy continues through the programs, relationships, and people who carry this work forward. This transition was designed to honor more than 45 years of impact and trust.
Why AllThrive 365? formerly Foundation for Senior Living (FSL)
AllThrive 365 is a respected, social impact organization committed to helping Arizonans age with confidence through health, housing, and connection. Together, our organizations reflect a collective 97 years of service to Arizona communities. AllThrive 365 stood out for its strong alignment in values, deep community roots, and proven ability to deliver programs that support health and independence.

What core service lines are included in this transition?
Are services being eliminated?
No. All three core programs will continue. There are no service reductions as part of this transition.
Impact on Service Recipients
Will my service change?
No. You will continue receiving the same support with the same commitment to dignity, compassion, and personal connection.
Will I (service recipients) need to reapply or change anything?
No. The process to receive services and the staff who provide the services remains the same.
The Duet phone number will automatically transfer you to the AllThrive 365 phone number of 602-285-1800.

Will my role change?
No. Your service remains essential and deeply valued. You will continue working with your current matches and supporting the same mission.
Donors & Supporters
How can I continue to provide support?
You can continue to direct your support to the same services you care about, in alignment with their intended charitable purposes. Donations to support homebound adults, family caregivers, and grandparents raising grandchildren can be made at allthrive365.givevirtuous.org/donate/duet.
Does this honor donor intent?
Yes. This transition is designed to honor donor intent as your generosity will continue to benefit the individuals and families these services are designed to serve.
Is AllThrive 365 a Qualified Charitable Organizations?
Yes. AllThrive 365 is a 501(c)3 social benefit organization and a Qualified Charitable Organization (QCO). Tax ID #86-0298945. QCO #20199.
Maximum QCO donation amounts for tax year 2026 (through the filing deadline of April 15, 2027): $506 single, married filing separate or head of household; $1,009 married filing joint.
